What to include in a residential rehab or detox referral

Clinical information

Strong referrals start with the clinical details our team needs to safely assess whether Passmores House is the right level of care.

GP summary

A detailed GP summary helps us understand someone’s medical history and plan their care appropriately. Useful summaries include height, weight, smoking status and any medication allergies. These details inform treatment planning and practical arrangements such as room allocation.

Recent blood test results

Please include a Full Blood Count and Liver Function Tests, even where the referral is for residential rehabilitation only. Results should be no more than two months old and help us confirm someone can enter treatment safely.

Clear treatment request

Please indicate the treatment pathway being requested. For example:

  • Detox and rehabilitation
  • Rehabilitation only
  • Stabilisation and transfer to Buvidal
  • Alcohol detox while remaining on opioid substitution treatment (OST)

Specifying the intended pathway helps us plan the most appropriate admission for the person you’re referring.

Risk and safeguarding information

Full risk and safeguarding information helps us maintain a safe environment for all residents while supporting people with complex needs.

Please include, where relevant:

  • Children in their care
  • Social worker involvement
  • Caring responsibilities
  • Any additional support needs

Please also disclose any relevant convictions or history of violent behaviour. This does not automatically prevent admission, but it helps us assess risk appropriately and ensure the right safeguards are in place from the outset.

Additional information

These details help us prepare in advance and ensure the environment is right for the person you’re referring from the moment they arrive.

Please include, where relevant:

  • Access needs: such as language support or literacy difficulties
  • Housing situation: to inform discharge planning if needed
  • Mobility needs: including use of aids or difficulty with stairs
  • Readiness for admission: for example, if they need time to arrange childcare, pet care or work commitments

Supporting documents such as hospital discharge summaries, CMHT reviews or hepatology reports can also be useful in helping us build a complete picture of someone’s needs.
